Please fill in the boxes on the form and later mail your deposit check to Lockport Marine, Inc.  We'll send you a confirmation of your reservation.

Please reserve a (fill in type) houseboat for the period from 1 to 3 p.m. (day, month, date) to 8:30 a.m. (day, month, date).

Boats may be boarded between 1 and 3 p.m. and must be back at Alma, Wisconsin dock on the date and time mentioned above.

The rental (certified or travelers checks or cash only) of $ plus 5 1/2% sales tax is to be paid before the boat is taken out.  This is in addition to the security deposit which is refundable if the boat is returned on time, clean and undamaged and with complete inventory.
